You chose to give a bolus of 20/kg normal saline over 5 minutes.
Sarah Jane is fitting, she also has severe respiratory distress and features of shock.
After insertion of an IV cannula you give the fluid. She continues fitting and as she does she becomes more cyanosed and eventually stops breathing. There is no noticeable response to the fluid. After about 10 minutes the fitting has stopped, but so has the heart.
You could always choose another initial option....
